For OB credentials at CHOSA: 15 cases are needed
For Neonate (less than 60 weeks old) credentials at CHOSA: 15 cases are needed

$210 /hr
$225 all inclusive for travel (must be outside of 60 mile radius)
Malpractice Insurance covered including tail coverage
Annual Profit sharing
Weekly direct deposit
Dedicated account manage Credentialing coordinator will facilitate credentialing
Coverage time:
6 months with extension option
